Outreach

Our rural population requires active efforts to reach our Elders and isolated patients who require assistance. In order to fulfill our stewardship to the Tule River Indian Reservation, our Community Outreach department is dedicated to serving the needs of these populations. Community Outreach Services include case management, diabetes & health education efforts, in-home assessment & referrals, patient transportation services, community health promotion, Elder services & nutrition, healthcare & patient advocacy, and community health education.

Outreach Services:

Diabetes Program:

Our Diabetes program under the coordination of Margaret Hernandez provides monthly activities such as walks around the gym, light exercise, and education. All with light lunch provided. Please click for more info: http://www.trihci.org/DiabetesProgram.html

Community Health Representative:

CHR’s PROUDLY PROVIDE *Blood Sugar checks, Blood pressure checks, health education, safety education, assistance with med refills and delivering medication ( to people with limited transportation and to the elderly)

Elder Services:

Charles Houston, our Senior Services Coordinator, runs our Elder Services Department. Some of the services provided are home delivered meals, supportive services, transportation, escort, legal services, light-housekeeping, visits, telephoning, family support, ombudsman, and caregiver support.

Transportation:

Our health center proudly services transportation for the community. To request transportation, you must meet the following criteria:

Transportation Requests

1. To qualify for Transportation Services, patients must be:

  • Contract Care eligible;
  • 18 years of age; or
  • Accompanied by an adult if under 18 years of age.

2. Patients who need transportation will be solely responsible for contacting the Outreach Transportation Department to schedule their transportation appointment.

  • Request for local transportation, including the Reservation and Porterville, must be received no less than 24 hours in advance.
  • Request for out of area transportation, such as areas other than Porterville or the Reservation, must be received no less than 2 weeks in advance.

3. The patient must contact the Outreach Transportation Department directly, or leave a voice mail message explaining he specific need.

4. Patient transportation requests will be prioritized as follows:

  • Those needing urgent transport as requested by a TRIHCI provider; or
  • Those without available means of transportation, including those without an automobile, those with an automobile that is unavailable due to a family member using it for employment, or those with an automobile that is not in proper running order.

5. Those who meet eligibility for transportation must have exhausted all other avenues before requesting transportation from the TRIHCI Outreach Department.

6. Individuals living off of the Reservation must first seek to obtain transportation from community resources such as family members or the public transportation system.
