At Tule River Indian Health Center, we believe care doesn’t stop at the clinic door. Through our Outreach Services, we extend health and wellness support to anyone who may need assistance getting to appointments, managing a condition, or finding local resources.

As part of our stewardship to the Tule River Indian Reservation, our Outreach team works to make healthcare more accessible and connected to community life.

Caring for the Community in the Community

Our goal is to make care simpler and closer to where you live. Here are some of the ways we help community members stay healthy and connected:

Diabetes Program

Monthly activities combine exercise, nutrition, and education, helping participants stay active and manage diabetes in a supportive environment.

Community Health Representatives

Our team provides home visits, medication support, health checks, and education for community members who need extra help managing their care.

Elder Services

We help elders remain safe and connected with meal delivery, supportive services, and caregiver assistance.

Transportation Assistance

If you need a ride to a medical appointment, our Transportation Program can help. Transportation is available for Tule River Reservation residents who qualify for Outreach Services.

Rides must be scheduled at least 24 hours in advance for local appointments and two weeks in advance for out-of-town visits. Riders must be 18 or older, or accompanied by an adult.

When you call, please be ready to share your appointment time, location, and any special needs so our team can plan your trip safely and on time.

Who can receive Outreach Services?

Outreach Services are available to residents of the Tule River Indian Reservation and tribal members who need help accessing healthcare, transportation, or in-home support. Our programs are designed to reach elders, families, and individuals who may have difficulty getting to the clinic or managing their care on their own.
